Clinical Characteristics and Diagnostic Evaluation of the Dizzy Patient


Abstract
The dianostic evaluation of the patient with dizziness needs careful history and complete otoneurologic examination, special tests of auditory and vestibular function and a radiologic imaging and other laboratory tests.
The author evaluated 209 dizzy patients(Male : 99, Female : 110) who visited the Department of Otolaryngology, Kangnam St. Mary¢¥s Hospital from January 1991 to December 1991.
@ES The results were as follows :
@EN 1. There was no different incidence of dizziness between male and female. The mean age of the studied subjects was 44.9 yrs and the age distribution was the highest in the 5th decade(28.2%).
2. The most common concomitant disease in the past history was hypertension(12.4%).
3. The most common character of dizziness was whirling(or spinning) sensation(64.5%).
4. The mode of onset was sudden in most cases(83.7%). The duration of dizziness was longer than an hour in 31.1% of cases and continuous in 26.8% of cases. The temporal profile of dizziness was episodic in 68.9% of cases. The severity of
symptoms
was
severe in 70.8%.
5. Hearing loss, tinnitus and ear fullness were accompanied in 35.4% of cases.
6. Abnormal electronystagmographic findings were found in 75.6% of cascs.
@ES These findings include :
@EN (1) Spontaneous nystagmus in 65 patients(31.1%)
(2) Gaze nystagmus in 28 patients(13.4%).
(3) Head shaking nystagmus in 102 patients(48.8%).
(4) Positional nystagmus in 38 patients(18.2%).
(5) Abnormal smooth pursuit in 4 patients(1.9%).
(6) Abnormal optokinetic nystagmus(OKN) and abnormal optokinetic after nystagmus(OKAN) in 11 patients(5.3%).
(7) Abnormal caloric responses in 76 patients(36.7%).
7. The majority of patients who had positive results(18.2%) in standing and deviation tests had peripheral disorders.
8. Central disorders were found in 3 patients(1.0).
9. In radiologic evaluation, 24 patients(11.5%) had abnormal findings in simple temporal bone X-ray, 11 patients(5.2%) had abnormal findings in C-T scan, 3 patients(1.4&) had abnormal findings in MRI.
10. In other laboratory test, 2 patients had positive results of VDRL test.
